The applicant must be a holder of a professional license from the Ministry of Health.

Procedures

  1. The applicant shall visit Dental Clinics and Centers Licensing Department and submit the application and the enclosed documents to cancel the license.
  2. The application is then documented in the register.
  3. The application will be referred to the Director of the Health Professions and Institutions Licensing Directorate who will review and transfer it to Dental Clinics and Centers Licensing Department.
  4. Upon approval, a letter of cancellation signed by the Minister of Health will be prepared.

  • The License Approval Letter, issued by the Ministry of Health.
  • A copy of the Civil Status ID Card.
